The Haase House's experienced and compassionate staff use individualized, goal-driven programming and unique hourly schedules to provide support to young adults with disabilities both in our home, clients' homes, and in the community. It’s through experiencing "real life" opportunities in a natural, safe home setting where individuals work towards building a brighter, more independent future for themselves--one focused on employment, community engagement, and daily living.

Our hope is for every young adult with a disability to have the opportunity to optimize post-high school services that bridge their current daily living skill set to a more self-sufficient, independent, and purposeful one. Within the walls of THH, our cozy ranch home in a quiet neighborhood in the Village of Mukwonago, we function like a family. We encourage each other when tasks are challenging, support and improve social interactions as they naturally occur, share in life’s ups n’ downs, and share new experiences together within our community. These experiences are invaluable when it comes to establishing positive, appropriate interactions that our participants can use in current and future relationships. Having a quiet, calming setting to provide instruction has proven to keep anxieties down and provides an optimal learning environment with opportunities for “space” and needed breaks.

Our structure is very different from the typical adult day-program in that we provide daily life-skill programming & instruction that is tailored to each individual’s goals. We work closely with families, individuals, guardians, school staff, employers, and caseworkers to assure we are providing specific, enriching, and self-determined activities that make progress toward each participant's life goals and greater independence. In our home setting, we have many opportunities to address social skills, strengthen participants' abilities for relationship-building, and encourage respectful engagement with others.
PERSONALIZED SCHEDULE DEVELOPMENT
Mary and THH staff communicate with each participant, their fiscal agent consultant, and their parent/guardian to develop a customized plan to ensure continued development in desired areas. The THH team takes several weeks to assess and determine baselines/goals, and each participant is considered in daily schedules to ensure their goals are being met.
INTEREST-BASED EVENTS & ACTIVITIES
We take our customization to the next level when planning our Special Events & Activities. Our schedule is driven by what our participants are interested in, what makes them happy, and what fulfills them. We also try to add unique events and experiences such as fishing, rock climbing, concerts, and pottery to build their options for independent leisure and recreation.

In addition to services that focus on skills training, we strongly believe that building relationships and a sense of community is critical for persons with disabilities. One popular aspect of THH is the “Special Events” we offer. These are offered to any young adult with a disability (18+) in the community at a very low cost per hour to cover support and transportation. Those enrolled at THH can use their Iong-term care funding for these events, as well. Special Events are intended to get our participants together with others in the community to broaden social opportunities, enjoy new activities, and build new, lasting relationships.
In addition to special events, we include opportunities to go into the community at least twice per day with the intent to build a deeper sense of community and teach participants how to best enjoy and utilize the community they are given. One of our major goals is to promote good citizenship and volunteerism to excite the community about engaging with and embracing persons with disabilities, encouraging greater acceptance and providing insight into participants' value and potential.
Hourly day services are provided in our home setting for 1-40 hours per week according to each participant's desired goals. Although “day services” often indicates "assistance" needed to meet daily living needs to assure adequate functioning and safe access to the community, our mission is to instruct and not just cue or prompt these individuals through daily activities. Our customized daily schedules include a wide range of opportunities in the home and community settings including gardening, cooking, exercise, social skills, hygiene practice, simple home repair, household chores, budgeting, hikes, simple woodworking, scrap booking, music exploration, relaxation, leisure exploration and functional academics (to name a few). The professional backgrounds of our staff allow us to design activities and curriculum that help meet the learning or physical needs of each individual participant.
Daily living skills services can occur in the client’s home, in the community, or at THH. DLS is provided by 1:1 or 1:2 instruction, and for this reason it is billed at a higher rate per hour. We encourage all participants to receive one hour per service day of DLS to help each participant move forward in the area they need the most intervention. We provide instruction and skill development to improve the participant’s ability to perform routine daily activities and effectively utilize community resources as independently as possible, focusing on potentials. DLS as defined through the Dept. of Health Services may include individual skill development in personal hygiene, food preparation, home upkeep/maintenance, money management, accessing and using community resources, computer use, parenting, and community mobility. In some situations, DLS may be used to provide support at a volunteer site or work setting in our community.
These services help participants obtain and maintain competitive or customized employment in a community-integrated work setting. This includes vocational/job-related discovery or assessment, person-centered employment planning, assistance with job placement, negotiations with prospective employers, job coaching, and tools/training to maximize the participant's effectiveness in that role. We work very closely with each business owner and family to assure the correct fit, training, and supervision for a successful outcome. THH may assist you in acquiring these services under your long-term care support funding. It is important to note that the participant’s case must be closed through DVR prior to being able to pursue THH for Supported Employment, and all options for support can be discussed further with each family.
Respite as it relates to THH is primarily used for Special Events/Outings outside of scheduled service times at THH. We bill our participants that attend THH under Respite only if they have enough funds to do so. Otherwise, THH participants without enough funds and other young adults with disabilities who do not receive Day Services, but want to build friendships, can attend all Special Events for a significantly reduced rate. Respite at THH may also be used if there is a need to arrive early or stay late due to unforeseen circumstances (i.e. car trouble, meeting before work, funeral to attend, etc…). This can occur in the client’s home, in the community, or at THH. In general, respite services temporarily relieve the primary caregiver from care demands. At THH, it means so much more.
These are educational services provided to the participants and caregivers to support and advance the development of their advocacy skills, exercise control and responsibility over their supported services, and address other needs. This includes enrollment fees, books and educational materials, structured meetings, information sharing, training, community visits, and so on as defined by the Dept. of Health Services. In our unique model, we have found this service code helpful to provide mentoring and support for IEP meetings, learning the concept of self advocacy and self determination, resolving employment issues, assisting with technology research, use and/or purchases, exploring other services available, and designing special curriculum or materials to deal with loss, family issues, relationships, behaviors, and so on.
We provide transportation at a reasonable and competitive cost as long as it does not disrupt specific individualized programming. Our priority is to maintain our day services schedule to the best of our ability. This means that we may not be able to offer a family our transportation services if the time and day they are needed does not fall into a natural route that our staff travel to come to Mukwonago or return to their homes at the end of their work days. We also will provide transportation to or from work if it does not disrupt services delivered. If we are providing support staff at the participant’s place of employment, transportation will certainly be provided if needed. We also provide all interested participants with information regarding other transportation available upon inquiring about our services. During a given day, we have the option to transport participants to all areas within our community and surrounding communities as part of their day services.
Funding put towards client programming allows us to bring in specialized instructors and expose our participants to a wider range of activities and potential job opportunities!
